Nose surgery - Primary rhinoplasty

Nose surgery - Primary rhinoplasty

Nose is the most prominent part of the human body.  The expression of the face depends on its size and shape. If you are not satisfied with your nose, it is possible to change its appearance with small and simple intervention, in accordance with your preferences and objective possibilities.

Some people have innately a large and deformed nose, while others can deform their nose in an accident or some kind of injury. Very often, external deformation of the nose is associated with the deformation of the internal structures, which leads to breathing difficulties. Both imperfections can be corrected with a single intervention.  During a nose surgery, subcutaneous structures are being changed; usually by bone and cartilage modeling and then adapting the skin to the new base.  Intervention is performed by internal incision, however, if necessary, small incision at the front nose septum is also possible. When modeling the nose, cartilage is sometimes added in order to correct some irregularities such as largely hanging tip or a crooked tip.

Some patients require the intervention only to recover  their airways, so called functional surgery, but without changing the external structures of the nose.

Examination and appointment:
“After the examination in our Surgical Center the procedure will be precisely described to you, with all the significant details. Each nose is individual and requires a personal approach. It is important that your nose matches your face after the surgery.  “
It is possible to estimate the outcome of the surgery by computer simulation in the Clinic.

Which is the bottom age limit for nose surgery?
It is best to wait until the bones and cartilages have completely grown. With girls it is usually about the age of 14 or 15 and with boys at about 15 or 16.
Why are photographs taken before the surgery?
“Photographs of patients in side view and front view are used during the pre- surgery consultations and for comparison with post- surgery results. They are filed in our archives.”

Is there pain after the surgery?
“Surprisingly, after the surgery the nose does not hurt, there is just the feeling of discomfort caused by the impossibility to breathe through the nose because of the nose tampons which are removed two or three days after the surgery.

Are there external scars?
"In majority of interventions we use internal incisions which are invisible. A little scar, only several mm long, is visible at open rhinoplasty. Open rhinoplasty is used for better access to nose structures without restrictions and with much better control. Even larger deformities may be resolved with such approach, mainly those of the tip of the nose as well as the deformities of the septum. This method is used at secondary rhinoplasties, i.e subsequent nose surgeries. Sometimes it is necessary to make the large nostrils narrower, which is done by incisions in the seam between the nose and the face where the scars are invisible.”

How long is the recovery process an when can one see the final results?
"Swelling and haematoma usually last for a week, in some cases a little longer. The result of the surgery may be seen once the nose mould is removed with only minor changes occurring up to a few months after the surgery when the healing process is completely terminated.  During the first few weeks after the surgery the tip and the ridge of the nose may be less sensitive to touch and the skin may become more greasy. After the surgery and the removal of tampons, the nose is stuffed for some time due to mucilage that remains in the nose. The nose should be blown very carefully. Local application of Sterimar drops is recommended.”

How long should the tampons and the shield be kept on the nose?
"Tampons are usually removed the second day after the surgery or, in case of more serious interventions on the septum, on the fourth day. The nose shield is made of aluminum and plastic material and is usually removed the seventh day after the surgery.”

Which are the possible complications?
“Serious complications are rare after nose surgery. One of the possible complications is nose bleeding after the intervention which may be controlled by an additional or subsequent tamponade (insertion of gauze or expanding materials). Infections are rare and treated with antibiotics. Sometimes a secondary intervention is necessary to correct the flaws of the first intervention, or to improve the breathing function. Other complications and problems are very rare and will be explained in more detail if you wish so.”

Who takes care of me after the surgery?
“While you are in the Polyclinic, we take care of you. After you are discharged, you can contact our Polyclinic in case you need to. After the surgery and discharge you will get an appointment to remove the tampons and shield from the nose”.

How long do I have to stay at the Clinic after the surgery?
“The surgery may be performed as an ambulatory intervention which means the patient may go home on the same day; or he/she may stay in the Polyclinic overnight”.

When can I go back to my everyday activities?
“Patients may engage in light work one week after the surgery. Straining should be avoided for two weeks because the increase of pressure after the strain may cause bleeding. Direct exposure to sun should also be avoided (you should use a sun screen with high protective factor). Wearing glasses is not recommended at first.”
